- A copy of a bank card/debit card of the same account number as the bank statements.
If you are self-employed, we ask that you attach the following documents to the completed registration form:
- Extract from the Chamber of Commerce (not older than 1 month);
- Accountant reports from the past two years (signed by accountant or bookkeeper);
- Copy of the last income/corporate tax return (IB60);
- Copy of bank statement business account (not older than 1 month);
- Copy accompanying bank card/debit card;
If you are retired, we ask that you attach the following documents to the completed registration form:
- Copy last tax statement(s) (pension, AOW);
- Copy bank statement of the last deposit(s) of your pension or AOW (etc.);
